A new chemo-free combo just cut death risk by 50% in head and neck cancer. The phase 3 LiGeR-HN1 trial found that adding petosemtamab to pembrolizumab slashed the risk of death by half and tripled response rates compared to pembrolizumab alone in patients with PD-L1–positive recurrent or metastatic head and neck squamous cell carcinoma (HNSCC). The results held regardless of HPV status, marking the first positive phase 3 trial in this population in several years.

The phase 3 LiGeR-HN1 trial delivered a major win for patients with recurrent or metastatic head and neck squamous cell carcinoma (HNSCC). Adding petosemtamab — a bispecific antibody targeting EGFR and LGR5 — to the immunotherapy pembrolizumab significantly outperformed pembrolizumab alone, meeting both primary endpoints of overall survival (OS) and objective response rate (ORR) at the very first interim analysis.
The combination works through three complementary mechanisms: blocking EGFR signaling, triggering receptor degradation in cancer stem-like cells, and pairing with pembrolizumab's immune checkpoint activity — all without chemotherapy. Importantly, the benefits were seen regardless of HPV status, and no new safety signals emerged.
By the Numbers
Why it matters: This is the first positive phase 3 trial in first-line recurrent/metastatic HNSCC in years, and it could redefine the standard of care by offering a chemotherapy-free regimen with dramatically improved survival and response outcomes. Regulatory submissions are expected to follow.
