Two dry eye drops are better than one. A real-world study found that combining Xiidra and Miebo significantly improved both signs and symptoms of dry eye disease in patients who had already tried multiple other treatments. Notably, 84% of patients were still using both drops nearly 200 days later — a strong signal for long-term tolerability and effectiveness.
For patients with stubborn dry eye disease, a two-drop combo may be the answer. A retrospective real-world study presented at Academy 2026 found that using Xiidra (lifitegrast) and Miebo (perfluorohexyloctane) together led to meaningful improvements in both objective signs and subjective symptoms — even in patients who had already cycled through four or more prior treatments.
The logic behind the combo: Xiidra targets inflammation, while Miebo reduces tear evaporation. By hitting both pathways simultaneously, clinicians saw improvements across the board — including fluorescein staining, tear breakup time, tear osmolarity, and patient-reported symptoms like eye pain, blurry vision, and light sensitivity. Adverse events were mostly mild to moderate.
Key Takeaways:
Why it matters: This real-world data fills a critical evidence gap for combination dry eye therapy, offering clinicians practical support for a multi-mechanism approach — and laying the groundwork for a potential single-drop solution that could simplify treatment for patients.
