A history of keratinocyte carcinoma (KC) — the most common type of skin cancer — is linked to more than four times the odds of developing mucosal melanoma (MM), a rare and aggressive cancer. A large case-control study using Veterans Health Administration and Mass General Brigham data found this association held across multiple mucosal sites. Clinicians may want to pay closer attention to mucosal sites during skin exams for patients with a KC history.
A new study published in the Journal of the American Academy of Dermatology suggests that patients with a prior history of keratinocyte carcinoma (KC) — the umbrella term for basal and squamous cell skin cancers — face a dramatically elevated risk of developing mucosal melanoma (MM), a rare cancer arising in mucous membranes like the nasal cavity, mouth, and genitals.
Researchers analyzed data from over 4,600 patients across the Veterans Health Administration and Mass General Brigham health systems (2000–2025), comparing MM patients against both cutaneous melanoma (CM) controls and melanoma-free controls. The KC-to-MM link held firm even after excluding lip cases and was consistent across multiple anatomical sites. Women and patients from certain racial/ethnic backgrounds — particularly Black and Hispanic patients — were also disproportionately represented in the MM group.

Why it matters: Mucosal melanoma is notoriously hard to catch early due to its non-specific symptoms and hidden locations. This study gives clinicians a concrete risk flag — a KC history — to prompt more vigilant mucosal site checks during routine skin exams, potentially enabling earlier detection of a cancer with poor prognosis.
