Menopause isn't the main culprit for women's rising blood pressure. A large Framingham Heart Study analysis found that women's pulse pressure starts climbing around age 37–39 — up to two decades before their final menstrual period — regardless of when menopause occurs. The findings suggest vascular aging in women is driven by more than just estrogen loss.

A major analysis of the Framingham Heart Study — tracking 6,760 participants over 14 years — found that women's pulse pressure (the gap between systolic and diastolic blood pressure) begins rising around age 37–39, well before menopause kicks in. Crucially, this timing was nearly identical whether women experienced early, average, or late menopause, challenging the long-held belief that estrogen loss is the primary driver of midlife arterial stiffening.
Researchers point to a different culprit: blunted aortic remodeling. While men's aortas continue expanding through their 80s, women's aortic growth plateaus earlier — and when combined with ongoing arterial wall stiffening, this may help explain why pulse pressure rises sooner in women.
