A tissue-free blood test can flag early breast cancer recurrence before symptoms appear. Studies show that detecting circulating tumor DNA (ctDNA) via a tissue-free epigenomic assay — no tumor sample needed — strongly predicts distant recurrence, with hazard ratios above 27 across multiple breast cancer subtypes. In one study, 94% of patients who tested positive went on to develop distant metastases.
Two new studies reinforce the power of tissue-free circulating tumor DNA (ctDNA) testing to predict breast cancer recurrence — no tumor tissue required. The Guardant Reveal assay, which analyzes blood plasma using epigenomic signals, was able to identify patients at high risk of relapse well before clinical symptoms emerged, offering a potentially game-changing surveillance tool.
In the larger study (SUCCESS-A trial, n=313), ctDNA was detected in 18 patients approximately 2 years after adjuvant chemotherapy. A striking 94% of those patients went on to develop distant metastases at a median of just 7.9 months after detection. A parallel analysis of 159 triple-negative breast cancer (TNBC) patients (c-TRAK trial) found the tissue-free assay performed comparably to tumor-informed approaches, with a median lead time of 7.6 months to recurrence and 92.5% concordance with multivariant tumor-informed assays.
By the Numbers
Why it matters: These findings suggest tissue-free ctDNA testing could help oncologists identify high-risk patients earlier and tailor interventions — without the logistical hurdles of obtaining tumor tissue. Prospective trials are still needed to confirm clinical benefit.
