Essential tremor, which affects about 2.2% of Americans, is now linked to significantly higher rates of mild cognitive impairment (MCI) and dementia than previously recognized. A large longitudinal study found that cognitive decline is a consistent feature of the disease — not just an incidental finding. Rates fall between those of the general population and Parkinson's disease patients.
Essential tremor has long been viewed primarily as a movement disorder, but new research published in Neurology makes a compelling case that cognitive decline is a core part of the disease. A prospective longitudinal study — the COGNET study — tracked 350 adults with essential tremor over up to 90 months, finding that rates of MCI and dementia were substantially higher than in the general population, though lower than in Parkinson's disease (PD) patients.
At baseline, 12.6% of participants had MCI and 6.3% had dementia. Over the course of follow-up, cumulative prevalence climbed to 22% for MCI and 14.6% for dementia. Researchers also found that the annual conversion rate from MCI to dementia was a notable 13.8%.
