A new prospective study found that excessive daytime sleepiness (EDS) in obstructive sleep apnea (OSA) patients is tied to measurable cognitive impairment and disrupted brain network connectivity, particularly in regions linked to attention and executive function. Altered activity in the middle frontal gyrus and inferior parietal lobule accounted for up to 42% of the sleepiness-related cognitive decline signal.
Feeling sleepy all day isn't just exhausting — it may be reshaping your brain. A prospective study published in the Journal of Magnetic Resonance Imaging found that excessive daytime sleepiness (EDS) in obstructive sleep apnea (OSA) patients is associated with measurable cognitive impairment, reduced glymphatic clearance, and disrupted functional brain network organization. The study enrolled 102 OSA patients and 86 healthy controls, using multiparametric MRI and a full neuropsychological battery to map the neurobiological toll of chronic sleepiness.
OSA patients scored significantly worse on tests of attention, processing speed, and executive function, and higher sleepiness scores tracked with poorer performance across all domains. Imaging revealed reduced subcortical glymphatic clearance and abnormal connectivity in the bilateral middle frontal gyrus (MFG) and left inferior parietal lobule (IPL) — both part of the default mode network. Mediation analyses showed these network disruptions partially explain how EDS translates into cognitive decline.
