People are buying unapproved retatrutide online and sharing their experiences on Reddit — and what they're reporting doesn't quite match clinical trial data. A new JAMA Network Open study analyzed nearly 150,000 posts and found increased appetite, fatigue, and insomnia topping the list, while GI symptoms — the hallmark of similar weight-loss drugs — took a back seat. Causation remains unconfirmed, but clinicians are urged to ask patients about unapproved peptide use.
Reddit is filling a safety surveillance gap for a drug that isn't even approved yet
Retatrutide is still in clinical trials and not approved anywhere in the world — but that hasn't stopped people from buying gray-market versions and posting about their experiences online. A new study published in JAMA Network Open analyzed nearly 150,000 retatrutide-related Reddit posts from 2021–2025 and found that the side effects users reported diverged notably from those seen in phase 2 trials.
While GI symptoms like nausea dominated clinical trial reports, Reddit users flagged increased appetite, fatigue, increased energy, and insomnia as their top complaints. The increased appetite finding was especially striking, given that retatrutide is being studied precisely for its weight-loss effects. Researchers used a large language model to classify posts and extract symptom data, with manual validation confirming high accuracy. Importantly, causality cannot be confirmed — product purity, dosing, and "stacking" with other peptides are all unverified variables.
By the Numbers
Why it matters: With Lilly planning an FDA Biologics License Application in early 2027, clinicians are increasingly likely to encounter patients already using unapproved versions. Experts recommend nonjudgmental conversations about gray-market peptide use and vigilance for symptoms beyond the typical GI profile.
