The goal: fewer ER visits, less disruption, and care that actually matches what older adults want and need.
SCAN Health Plan is partnering with the Springs Living, a senior living organization, to roll out co-branded health plans for older adults in Oregon's Clackamas and Washington counties — marking SCAN's first expansion into the state. Available January 1, the plans are built around an institutional special needs plan (I-SNP) model that connects residents' everyday assisted living support directly with their medical care.
The idea is straightforward but meaningful: instead of shuttling vulnerable seniors to outside appointments, bring the care to them. Think podiatrists visiting memory care residents on-site, or clinicians getting real-time updates from the caregivers who know patients best. SCAN CEO Sachin Jain, M.D., says the default of "head to the ER every time there's a problem" has long failed this population — and this model is designed to change that.
