The Trump administration has been touting its Most Favored Nation (MFN) drug pricing deals with 26 pharmaceutical companies as a win for American patients — but a closer look reveals a significant hidden tradeoff. According to an updated analysis from Mass General Brigham researchers, these agreements could reduce projected Medicare savings by nearly 80%, because companies that signed deals believe they're exempt from two separate Medicare pricing models already in the pipeline.

Those models — GLOBE (for Medicare Part B) and GUARD (for Medicare Part D) — were designed to peg U.S. drug prices to those in comparable countries, with a projected savings of $11.6 billion per year in their initial phase. But if companies with MFN agreements are carved out, a substantial chunk of those savings evaporates. The federal government hasn't officially confirmed which companies will be exempt.

Even beyond the Medicare angle, experts warn that lower manufacturer prices don't automatically translate to savings for patients, given the complex web of contracts between drugmakers, insurers, pharmacy benefit managers, and pharmacies. The administration's direct-to-consumer platform, TrumpRx, offers some cash-pay discounts, but only covers 92 of 800 brand-name drugs from participating companies.
