ASCO has released its first living guideline on the systemic treatment of recurrent high-grade serous and/or endometrioid epithelial ovarian, fallopian tube, or primary peritoneal cancer — published in the Journal of Clinical Oncology. A multidisciplinary expert panel screened 819 publications, with 36 randomized controlled trials and systematic reviews directly informing 17 recommendations across six research questions covering systemic therapy, maintenance, surgery, and post-treatment monitoring.
For platinum-sensitive recurrence (>6 months from last platinum exposure), carboplatin doublets — paired with PLD, paclitaxel, or gemcitabine — remain the standard of care. Bevacizumab can be added and continued as maintenance to improve progression-free survival, though no overall survival benefit has been demonstrated. For platinum-resistant or refractory disease, mirvetuximab soravtansine is now the preferred option for FRα-high tumors, backed by the MIRASOL trial's improvements in both PFS and OS.
