GLP-1 receptor agonists, best known as weight-loss drugs, may have direct anti-inflammatory powers that work independently of shedding pounds. Emerging evidence points to the brain-immune axis as a key mechanism, with implications for diseases like psoriasis, psoriatic arthritis, and even long COVID. Experts say these drugs could one day be used as broad anti-aging therapies.
GLP-1 receptor agonists — the class of drugs behind blockbusters like Ozempic and Zepbound — may do a lot more than help people lose weight. At the Congress of Clinical Rheumatology West, Cleveland Clinic immunologist Dr. Leonard Calabrese presented a compelling case that these drugs have direct anti-inflammatory effects, potentially mediated by the brain-immune axis, and that those effects can show up even before significant weight loss occurs.
The evidence comes from multiple fronts. In the TOGETHER-PsO trial, patients on combination GLP-1/ixekizumab therapy showed skin clearance improvements over monotherapy as early as week 4 — when weight loss was still minimal. A separate 2022 study found that weight loss and glucose control explained only ~60% of CRP reductions in type 2 diabetes patients on GLP-1 therapy, suggesting an independent anti-inflammatory pathway. A landmark 2024 Cell Metabolism study using mouse models showed that GLP-1's anti-inflammatory effects require central neuronal receptors — pointing to the brain as a key driver.
