Two biologics — dupilumab and mepolizumab — are showing meaningful, sustained improvements for patients with severe chronic rhinosinusitis with nasal polyps (CRSwNP) in real-world studies. Dupilumab also eased ear inflammation and improved middle-ear function in patients with co-occurring eosinophilic otitis media. Both drugs reduced polyp burden, improved quality of life, and relieved symptoms over 12–24 months.
Two biologics are racking up real-world wins against stubborn nasal polyps
Patients with severe, uncontrolled chronic rhinosinusitis with nasal polyps (CRSwNP) are seeing meaningful relief from two biologics — dupilumab and mepolizumab — across multiple real-world studies. Dupilumab showed sustained improvements in nasal congestion, smell, and quality of life through 24 months in a global registry of 691 patients, with the share reporting "no symptoms" jumping from 3.3% at baseline to nearly 43% at 24 months. Mepolizumab delivered comparable gains in polyp burden, sinonasal symptoms, and asthma control across two separate real-world cohorts, with patients who had comorbid asthma seeing even greater benefits.
A standout finding: dupilumab also improved outcomes in patients with eosinophilic otitis media (EOM) — a difficult-to-treat middle-ear condition that often co-occurs with severe CRSwNP. In a 48-patient study, quality-of-life scores for ear disease dropped dramatically, and 75% of patients with abnormal middle-ear pressure readings normalized by 12 months.
