Guiding the needle in real time makes a big difference. A randomized trial found that real-time ultrasound guidance during thoracic epidural puncture achieved significantly higher first-pass success rates compared to simply marking the spot beforehand. The technique also reduced the number of needle passes needed, with no increase in serious complications.
Getting an epidural right the first time matters — and a new study suggests real-time ultrasound guidance is the way to do it. A randomized controlled trial conducted in Japan compared two ultrasound approaches in 64 adults undergoing abdominal surgery requiring thoracic epidural anesthesia: real-time ultrasound guidance during needle advancement versus preprocedural ultrasound marking alone (done before needle insertion). The verdict? Real-time guidance came out on top.
Patients in the real-time group had significantly higher first-pass success rates and required fewer needle passes overall. Procedure time was similar between the two groups, and neither group experienced serious complications like epidural hematoma, abscess, or dural puncture at the two-week follow-up mark.
