An inflammatory marker called IL-6 may be a powerful predictor of heart failure risk in adults with type 2 diabetes and overweight or obesity. A post hoc analysis of the Look AHEAD trial found that higher IL-6 levels were linked to a 43% increased risk of heart failure — and that an intensive weight-loss program significantly reduced those levels within just one year.
A new analysis of the landmark Look AHEAD trial suggests that interleukin-6 (IL-6) — an inflammatory protein — could be a meaningful predictor of heart failure (HF) risk in adults with type 2 diabetes and overweight or obesity. Researchers found that higher baseline IL-6 concentrations were significantly associated with incident heart failure over a 12-year follow-up, even after adjusting for multiple variables including statin use. Notably, high-sensitivity C-reactive protein (hs-CRP), another commonly used inflammatory marker, did not independently predict HF risk.
The good news? An intensive lifestyle intervention targeting at least 7% weight loss through diet and exercise led to a significantly greater reduction in IL-6 levels at one year compared to standard diabetes education — suggesting that lifestyle changes can meaningfully dial down this inflammatory risk.
These findings challenge the traditional view that residual inflammatory risk is only relevant to atherosclerosis. For clinicians managing patients with type 2 diabetes, IL-6 may offer a more targeted window into heart failure risk — and lifestyle interventions may be a practical tool to reduce it.
