The vaginal microbiome is an underappreciated hotspot for antibiotic resistance genes (ARGs), especially during pregnancy. A large metagenomic study found that the type of bacterial community in the vagina strongly predicts resistance gene burden — and certain profiles are linked to serious pregnancy complications like preterm premature rupture of membranes (PPROM).
A new study published in npj Biofilms and Microbiomes analyzed nearly 2,000 vaginal metagenomic samples from 1,547 pregnant women in France — and found that more than half harbored clinically relevant antibiotic resistance genes (ARGs). The key driver? The type of bacterial community living in the vagina.
Women with Lactobacillus crispatus-dominated communities (CST I) were largely in the clear — 75% of those samples were ARG-negative. But women with L. iners-dominated (CST III) or anaerobe-rich communities (CST IV) carried significantly higher resistance burdens. Cases of preterm premature rupture of membranes (PPROM) showed the broadest predicted resistance profiles, alongside enrichment of opportunistic bacteria like E. coli.
Why it matters: With approximately 19 million maternal-fetal infections occurring globally each year, understanding how the vaginal microbiome shapes antibiotic resistance is critical. These findings raise important questions about whether resistance genes can be transmitted from mother to newborn — and whether supporting a L. crispatus-dominant microbiome could become a meaningful target in perinatal care.
