A phase 2 trial found that adding venetoclax to DA-EPOCH-R chemotherapy in newly diagnosed double-hit lymphoma patients made things significantly worse — not better. Median progression-free survival dropped to 7.7 months with venetoclax vs. 28.4 months without it, and treatment-related deaths were far more common in the venetoclax group. The trial was closed early due to safety concerns.
Researchers hoped that adding venetoclax — an oral BCL2 inhibitor with known activity in some B-cell lymphomas — to the intensive DA-EPOCH-R chemotherapy regimen would improve outcomes for patients with newly diagnosed double-hit lymphoma, a particularly aggressive cancer. Instead, a randomized phase 2 trial published in The Lancet Haematology found the combination was both less effective and significantly more dangerous.
The trial enrolled 66 patients and was closed early due to safety concerns. Patients receiving venetoclax plus DA-EPOCH-R fared considerably worse across nearly every measure compared to those on DA-EPOCH-R alone.

This trial delivers a clear warning: venetoclax not only fails to help in this setting but actively harms patients, primarily through increased infection-related and cardiac deaths.
