The model, trained on nearly 2,500 veterans with metastatic hormone-sensitive prostate cancer, uses 16 cancer-associated gene mutations to sort patients into favorable, intermediate, and unfavorable risk groups.
The survival differences are striking: men with favorable profiles had a median overall survival of 87 months, compared to 52 months for intermediate and just 31 months for unfavorable classifications. The findings held up in a separate non-veteran validation cohort, lending credibility to the tool's broader applicability. Researchers say the next step is a clinical trial testing whether men with favorable genomic profiles can safely de-escalate androgen deprivation therapy — a treatment known for significant side effects including cardiovascular risk.
