Retrograde cricopharyngeus dysfunction (R-CPD), or "no burp syndrome," has gone from an obscure curiosity to a formally recognized GI condition. Once dismissed or misdiagnosed for years, it's now included in the Rome V diagnostic criteria. A Botox injection into the cricopharyngeus muscle relieves symptoms in about 90% of patients, offering life-changing relief for thousands who previously had no answers.
For decades, people who couldn't burp were told their test results were normal — or worse, laughed out of their doctor's office. Now, retrograde cricopharyngeus dysfunction (R-CPD), informally known as "no burp syndrome," has earned formal recognition in the Rome V international diagnostic criteria for gut-brain interaction disorders. The condition, first described in medical literature in 1987 and named in 2019, occurs when the cricopharyngeus muscle fails to relax, preventing the upper esophageal sphincter from opening to release gas. The result: bloating, chest pain, constipation, and a telltale gurgling sound — symptoms that often led to years of misdiagnosis as IBS, GERD, or SIBO.
Awareness has surged largely thanks to internet forums. A 2015 Reddit post went viral, eventually growing into a 16,000-member subreddit (r/noburp) and flooding specialists with patients. The go-to treatment — injecting 25–100 units of Botox into the cricopharyngeus muscle — boasts a ~90% success rate, with effects often outlasting the Botox itself.
