OnabotulinumtoxinA (Botox) meaningfully cut headache days and resolved medication-overuse headache in chronic migraine patients who had already failed multiple treatments. A real-world study of 479 patients found over 27% achieved at least a 50% reduction in monthly headache days after 9 months. However, a family history of migraine and four or more prior treatment failures predicted a weaker response.
A new retrospective study offers encouraging real-world evidence that onabotulinumtoxinA (BoNT-A, or Botox) can meaningfully reduce the burden of chronic migraine — even in patients who've already tried multiple other treatments without success. Conducted across 12 tertiary headache centres in Poland, the study tracked 479 patients (nearly 89% women, mean age 43.6) who received BoNT-A injections every 3 months for 9 months under a national reimbursement programme.
After 9 months, patients saw significant drops in monthly headache days, migraine days, and acute medication use — and over 41% who had medication-overuse headache (MOH) no longer met the criteria for it. Still, the authors flagged a key concern: delaying advanced therapies like BoNT-A until after multiple treatment failures may limit how well it ultimately works.
Why it matters: For the millions living with chronic migraine that doesn't respond to standard preventive therapies, this study reinforces BoNT-A as a viable option — while also highlighting that earlier escalation of care could be key to maximising its effectiveness.
