A study published in the American Journal of Epidemiology analyzed data from 2,514 children across 23 U.S. states and found that prenatal exposure to fluoride in public drinking water at or above 0.675 mg/L — just below the U.S. Public Health Service's recommended optimal level of 0.7 mg/L — was associated with slightly lower fluid reasoning scores in children. For every additional 0.1 mg/L of fluoride above that threshold, scores dropped by 0.67 points. No significant associations were found for crystallized cognition.
Experts are urging measured interpretation. The study doesn't establish causation, and residual confounders — like socioeconomic disparities and co-exposure to arsenic and uranium — may explain the results. Several other large studies from Australia, New Zealand, Europe, and the U.S. found no cognitive impact at recommended fluoride levels. The American Dental Association, American Academy of Pediatrics, and other major health bodies continue to endorse community water fluoridation as safe and beneficial.
