A large longitudinal study of over 7,600 women found that TSH levels rose gradually with age but showed no abrupt shift at the menopausal transition. Clinicians are advised to focus on age-related thyroid monitoring rather than tying screening to menopausal status.
Thyroid problems in midlife women may have less to do with menopause than previously thought — and more to do with simply getting older. A large longitudinal study published in The Journal of Clinical Endocrinology & Metabolism tracked over 7,600 healthy Japanese women across natural menopause and found no distinct shift in thyroid-stimulating hormone (TSH) or free thyroxine (fT4) levels at the menopausal transition.
TSH levels did rise gradually over time, and the prevalence of both hypothyroidism and hyperthyroidism increased with age — but neither showed a meaningful change in trend specifically at menopause. The authors conclude that aging, not the hormonal shift of menopause, is the primary driver of thyroid dysfunction risk in this population.
