A Scottish cohort study found distinct DNA methylation patterns in blood up to 8 years before an ulcerative colitis diagnosis — and a predictive model built from these signals performed impressively. The findings could pave the way for early disease interception strategies.
Researchers have identified specific DNA methylation changes in whole blood that appear years before a clinical diagnosis of ulcerative colitis (UC) — offering a potential window for early intervention. Using data from a large Scottish prospective cohort, scientists detected 16 distinct epigenetic changes in individuals who later developed UC, with blood samples collected a median of 8 years before diagnosis. Crucially, these changes didn't intensify as diagnosis approached, suggesting they represent stable early biological signals rather than a disease progression marker.
A predictive model built from these methylation signatures showed strong performance, correctly distinguishing future UC patients from those who went on to develop irritable bowel syndrome or rheumatoid arthritis — pointing to the disease-specific nature of these early changes.
