Rapid ART initiation keeps newly diagnosed HIV patients engaged in care during the critical first months. A prospective cohort study found that same-day ART starters had significantly higher retention at 3–4 months (50% vs. 27%) and reached viral suppression nearly three times faster than those who delayed. However, these advantages largely faded by the one-year mark.
Getting newly diagnosed HIV patients on antiretroviral therapy (ART) right away appears to make a real difference — at least in the short term. A prospective cohort study out of Louisville, Kentucky, found that patients who started ART on the same day as their diagnosis were significantly more likely to stay engaged in care at 3–4 months and reached viral suppression more than twice as fast as those who waited.
The study followed 168 treatment-naive adults newly diagnosed with HIV between 2021 and 2023. About 64% started ART on the day of diagnosis (the "rapid" group), while the remaining 36% followed standard intake procedures after presenting more than a week post-diagnosis.

Why it matters: The early window after an HIV diagnosis is critical — both for the patient's health and for reducing transmission. While rapid ART's benefits appear to taper off by year one, the authors suggest that pairing immediate treatment with sustained supportive services could help preserve those early gains over the long term.
