Physical events like surgery, injury, or immobilization in the two weeks before a stroke may significantly raise the risk of cryptogenic ischemic stroke (CIS) in young adults — especially those under 40 or with a patent foramen ovale (PFO). A new multicenter case-control study found these mechanical venous thrombosis risk factors were more common in young stroke patients compared to stroke-free peers, underscoring the need to ask about recent physical triggers during stroke workups.
A new study published in Stroke suggests that physical events — think surgery, injury, or prolonged immobilization — occurring within two weeks before a stroke may be an underappreciated trigger for cryptogenic ischemic stroke (CIS) in young adults. Researchers analyzed 546 patients aged 18–49 with CIS and matched them with stroke-free controls, finding that these mechanical venous thrombosis risk factors (VTRFs) were significantly more common in stroke patients and independently associated with CIS.
The link was especially pronounced in adults aged 18–39 and in those with a clinically relevant patent foramen ovale (PFO) — a small hole in the heart that can allow clots to pass from the venous to arterial circulation. Notably, the association disappeared when the triggering event occurred 15–90 days before stroke, suggesting a narrow but critical risk window.
