GLP-1 medications have surged in popularity, with 11% of U.S. adults now using them for weight loss — up from 3% in 2024. But new data show a massive treatment gap: 76% of people actively trying to lose weight have never used a prescription weight-loss medication. Cost, insurance coverage, and systemic disparities continue to keep these life-changing drugs out of reach for many who need them most.
GLP-1 receptor agonists have become a cultural phenomenon, with awareness now at 91% among U.S. adults — but awareness doesn't equal access. Two new reports reveal a striking paradox: while GLP-1 use has tripled since 2024, the majority of people actively trying to lose weight have never used a prescription weight-loss medication. Among those untreated, 76% had a BMI ≥ 25 and 39% had a BMI ≥ 30 — meaning many who could clinically benefit are being left behind.
The gaps aren't random. Use is concentrated among women, adults aged 40–59, and higher-income households. Those earning ≥ $150,000 were nearly twice as likely to use GLP-1s as those earning under $25,000. Meanwhile, 35% of compounded GLP-1 users had switched from brand-name products — mostly due to cost or insurance — raising quality and safety concerns. The American College of Physicians is now calling on all payers, including Medicare and Medicaid, to cover evidence-based obesity medications.
