Recurrent cystitis could be a clinical red flag for urologic cancer, especially in the short term. A large Danish study found that patients with hospital-diagnosed recurrent cystitis had nearly five times the cancer risk of the general population within the first year. Women faced a persistently elevated long-term risk, particularly for bladder cancer — and those with five or more UTIs within six months showed a 2.5-fold higher likelihood of developing it.
Recurrent cystitis isn't just a nuisance — it may be a clinical marker for urologic cancer, particularly in the short term. A large population-based Danish cohort study of 42,377 adults found that patients with a first-time hospital diagnosis of recurrent cystitis had nearly five times the urologic cancer risk of the general population within the first year. Bladder cancer carried the steepest short-term signal, with a standardized incidence ratio (SIR) of 8.68 at one year.
While overall cancer risk normalized beyond two years for most patients, women remained at persistently elevated risk — with an SIR of 1.47 for all urologic cancers and 1.40 specifically for bladder cancer. A separate analysis reinforced this, finding that five or more UTI episodes within six months conferred a ~2.5-fold higher likelihood of bladder cancer, with a stronger dose-response relationship seen in women.
