The STAREE trial shows statins work for older adults too. Adults aged 70+ with no cardiovascular history who took 40 mg of atorvastatin daily had a 30% lower risk of major adverse cardiovascular events compared to placebo over nearly 6 years. Experts say the findings should prompt guideline updates — though statin use should still be tailored to individual patients.
The STAREE trial, presented at the European Society of Cardiology Congress 2026 and published in the New England Journal of Medicine, delivers a compelling case for statin use in older adults. Adults aged 70 and older with no prior cardiovascular disease who took 40 mg of atorvastatin daily saw a 30% reduction in major adverse cardiovascular events (MACE) — including CV death, nonfatal MI, stroke, and coronary revascularization — compared to placebo over a median follow-up of nearly 6 years.
That said, there was no difference between groups in disability-free survival (a composite of death, dementia, or persistent physical disability), meaning statins reduced events but didn't preserve independence. Experts note this makes life expectancy conversations critical — patients unlikely to survive 5+ more years, or who are already frail or managing multiple medications, may see limited benefit.
Why it matters: Current ACC/AHA guidelines don't specifically recommend statins for older adults, and the USPSTF calls the evidence insufficient for those 76+. STAREE challenges that gap — but experts stress decisions should still weigh life expectancy, frailty, polypharmacy, and patient priorities.
