When two doctors listen to the same child's lungs, they often hear something different. A new JAMA Network Open study found that clinicians examining children with pneumonia rarely agreed on key physical findings like crackles or decreased breath sounds — raising concerns about over-reliance on auscultation alone. Experts say the stethoscope still matters, but shouldn't be the only tool in the diagnostic toolbox.
Two doctors, one patient, two different conclusions — that's the diagnostic dilemma highlighted in a new study published in JAMA Network Open. Researchers found that clinicians examining the same children with community-acquired pneumonia frequently disagreed on what they heard and observed, suggesting that physical exam findings may be far less reliable than previously assumed.
The study analyzed 252 children across seven emergency departments between 2023 and 2025. Two clinicians independently examined each patient within an hour of each other, assessing eight physical findings. Agreement was measured on a 0–1 scale, with 0.61 considered substantial consensus — a threshold no finding came close to reaching.
