If a patient has a fluoroquinolone allergy label, the instinct is often to avoid the entire antibiotic class. But a new large-scale study suggests that approach may be overly cautious. Researchers analyzed data from over 250 healthcare organizations and found that cross-drug reactions among the three most common fluoroquinolones — ciprofloxacin, levofloxacin, and moxifloxacin — were rare, and the increased risk for certain drug pairs was only modest.

Notably, some drug pairs showed no increased risk at all. Most reactions were mild (pruritus or urticaria), and 70% occurred within a year of the initial allergy label. The authors suggest that patients labeled allergic to ciprofloxacin or levofloxacin may safely tolerate moxifloxacin — especially if the allergy is more than a year old.
