Interim trial data show the vaccine, paired with Keytruda, significantly reduced melanoma recurrence after surgery. Manufacturing complexity and limited survival data remain key hurdles before it could reach widespread use.
Moderna and Merck's personalized mRNA cancer vaccine, intismeran autogene, is generating excitement as a potential new frontier in oncology. The vaccine works by sequencing a patient's surgically removed tumor, identifying up to 34 unique cancer mutations, and creating a customized mRNA vaccine to train the immune system to recognize and destroy those specific cancer cells — used alongside Merck's immunotherapy Keytruda.
Interim data from a large ongoing trial, released August 19, showed the combination met its primary goal of reducing melanoma recurrence and its secondary goal of preventing metastatic spread, compared to Keytruda alone. Earlier five-year data from a mid-stage trial showed the vaccine cut relapse risk by half and reduced metastatic spread risk by 59%.

Why it matters: The vaccine's highly personalized nature makes manufacturing far more complex than traditional vaccines — each must be individually designed, produced, and tracked per patient, requiring a 10–100x scale-up for commercial use. Overall survival data are still pending, and it remains unclear whether the technology will work beyond highly mutated cancers like melanoma. Trials in lung, bladder, kidney, pancreatic, and stomach cancers are underway, with key data readouts expected in the next one to two years.
