Dilated pupils could skew the math on cataract surgery. A new study found that pupil dilation increases anterior chamber depth and reduces lens thickness — changes that can affect intraocular lens (IOL) power calculations. Surgeons are being urged to factor in a patient's pupil status before ordering preoperative biometry measurements.
When it comes to cataract surgery prep, timing might matter more than previously thought. A new study in the Journal of Refractive Surgery found that pupil dilation — commonly used during preoperative eye exams — can meaningfully alter the ocular measurements used to calculate intraocular lens (IOL) power. Since biometry and IOL calculations are often done on the same day as the surgical decision, patients' pupils are frequently still dilated when measurements are taken.
Researchers reviewed data from multiple studies and found that dilation caused a statistically significant increase in anterior chamber depth and a reduction in lens thickness. These shifts were enough to affect IOL power predictions for at least two widely used formulas — Barrett Universal II and Haigis — though six other formulas showed little to no impact.
