Antibiotic resistance in children is climbing fast globally, and the drugs we rely on most for serious infections are increasingly failing. A sweeping study of 106,000+ pediatric bacterial isolates from 82 countries found that resistance to "Watch" and "Reserve" antibiotics — the heavy hitters saved for severe cases — has more than doubled in ICUs over the past two decades. Without action, projections suggest things will get significantly worse by 2035.
Antibiotic resistance in children is rising at an alarming pace worldwide, and the drugs reserved for the most serious infections are bearing the brunt. A landmark study published in JAMA Pediatrics analyzed over 106,000 bacterial isolates from children in 82 countries between 2004 and 2022, finding that resistance to WHO's "Watch" and "Reserve" category antibiotics — the second- and last-line treatments — has surged, particularly in ICUs and among children with sepsis.
The problem is global but hits hardest in resource-limited regions. In Africa and Southeast Asia, resistance to last-resort antibiotics in Klebsiella species has already reached 33–43%. Meanwhile, even high-income countries like those in North America and Europe are seeing some of the sharpest increases in resistance rates. Experts warn that bacteria don't respect borders, and rising global resistance will inevitably affect wealthier nations too.

Why it matters: Children have been largely invisible in AMR surveillance — most data comes from adults. This study is a wake-up call for clinicians, policymakers, and public health officials to build pediatric-specific stewardship programs, expand rapid diagnostics, and invest in prevention strategies like vaccination and infection control before the 2035 projections become reality.
