The retrospective study of 2.2 million people found that over 70% of eligible residents participated, and more than 6,000 deaths were estimated to have been prevented. A landmark 21-year retrospective study out of Spain's Basque Country has delivered striking evidence for population-based colorectal cancer (CRC) screening. After launching a biennial fecal immunochemical test (FIT) program in 2009 targeting adults aged 50–69, the region saw CRC mortality drop by 50.1% in that age group — and an overall decline of 29.6% across all ages. A key inflection point emerged in 2012, just three years after the program began, when mortality rates shifted from rising to falling annually.
The program's success wasn't just about the test itself — participation was remarkably high. Over 70% of eligible residents completed FIT screenings throughout the study period, with more than 91% of those with abnormal results following up with colonoscopy. Of the cancers detected through screening, nearly 71% were caught at stage I or II, when treatment is far more effective.
