Lung transplant dramatically improves survival in a rare subset of stage IV NSCLC patients. A prospective JAMA study found 100% 1-year survival among 17 patients who underwent bilateral lung transplant for medically refractory, lung-limited disease — compared to just 40.8% among those managed medically. Experts caution this applies only to a highly select group and that longer-term data are still needed.
For decades, active lung cancer was considered a hard contraindication for lung transplantation. A new prospective study published in JAMA is challenging that assumption — at least for a very specific, rare group of patients. Researchers from Northwestern Medicine's DREAM program tracked 404 adults with end-stage pulmonary disease, including 98 with medically refractory, lung-limited stage IV non-small cell lung cancer (NSCLC). Of those, 17 underwent bilateral lung transplant while 81 received standard medical management due to nonbiologic barriers like financial or geographic constraints.
The results were striking: 100% of transplant recipients were alive at 1 year, compared to just 40.8% of those on medical management alone. Transplant recipients also showed comparable surgical outcomes to 306 non-cancer patients who underwent transplant for end-stage pulmonary disease. Four transplant recipients developed cancer recurrence, and two had died by January 2026 on extended follow-up.
