For two decades, compounded off-label bevacizumab has been the workhorse of retinal disease treatment — cheap, effective, and widely embraced by payers. That changed in July 2026 when the FDA approved Lytenava (Outlook Therapeutics), the first eye-ready bevacizumab formulation, specifically for neovascular age-related macular degeneration (nAMD). But the path to widespread adoption is anything but clear.
The core tension is cost. Off-label compounded bevacizumab runs about $75 per injection, while branded anti-VEGF alternatives range from $255 to nearly $2,400. Outlook hasn't set a US price yet, but European list prices hover around $637–$800 — still far above the compounded option. With up to three-quarters of commercial and Medicare Advantage plans using step therapy that mandates bevacizumab first, payers may continue favoring the cheaper compounded version, leaving Lytenava's market position uncertain.
Safety concerns about compounded bevacizumab — including endophthalmitis outbreaks, batch inconsistency, and silicone oil droplet contamination — have long simmered in the field. Lytenava offers a standardized, traceable alternative, but retina specialists note the frequency of serious complications has been low, even if the consequences can be severe.
