The Brooklyn-based value-based care company announced a definitive agreement to acquire Homeward Health in an all-stock deal, pairing Cityblock's AI-powered care-orchestration platform with Homeward's rural provider network and Medicare Advantage expertise. Simultaneously, Cityblock closed a $116M Series E funding round led by General Catalyst, bringing its total fundraising to over $900M.
Combined, the two companies will serve nearly 250,000 members and aim to reach the 120 million Americans enrolled in government-funded healthcare. The deal is particularly timely: roughly 60 million Americans live in rural areas, and 80% of rural counties lack sufficient primary care providers. Homeward, founded in 2022, has served more than 50,000 rural patients across 50 counties through mobile care units, in-home monitoring, and virtual care.
