Getting type 1 diabetes (T1D) young may come with a silver lining. Children diagnosed before age 7 had significantly lower risks for heart, eye, nerve, and kidney complications in the first 20 years after diagnosis compared to teens diagnosed at 13–15. But by age 35, most of that protection fades — except for kidney disease in those diagnosed between ages 7 and 12.
Two new studies shed light on how age at diagnosis shapes the long-term complication risk in childhood-onset type 1 diabetes (T1D). The larger study, led by researchers at the University of Sydney and published in Diabetes Care, analyzed data from over 5,200 children diagnosed with T1D before age 16. It found that those diagnosed before age 7 had dramatically lower risks for cardiac, eye, nerve, and kidney complications in the first 20 years after diagnosis compared to those diagnosed at ages 13–15.
However, that protective window doesn't last forever. By age 35, complication risks largely equalized across all age-at-onset groups — with one exception: those diagnosed between ages 7 and 12 retained a lower risk for kidney complications. A Swedish cohort study with 30+ years of follow-up reinforced the picture, finding that over 80% of childhood-onset T1D patients eventually developed diabetic retinopathy, and nearly half developed albuminuria.

Why it matters: These findings suggest that early-onset T1D buys time — but not a free pass. Clinicians should tailor long-term monitoring strategies based on age at diagnosis, and remain vigilant as patients approach their mid-30s, when complication risks converge regardless of when diabetes began.
