Kids with complex conditions are getting far more antibiotics. A study of 2.3 million children found that those with three or more complex chronic conditions had over 10 times the odds of high antibiotic use compared to healthy peers. The findings suggest medical complexity itself — not just specific infections — may be driving prescription rates.
A large retrospective study published in The Journal of Pediatrics reveals a striking link between medical complexity and antibiotic exposure in children. Analyzing data from more than 2.3 million Medicaid-enrolled children across 11 US states, researchers found that antibiotic prescription rates climbed sharply with the number of complex chronic conditions — from 514 per 1,000 in healthy kids to 2,882 per 1,000 in those with three or more complex conditions.
Beyond raw prescription rates, children with three or more complex conditions were also far more likely to receive a broader and more potent mix of antibiotics. While penicillins, cephalosporins, and macrolides dominated prescriptions in healthy children, kids with the highest complexity had notably higher rates of sulfonamides, quinolones, and aminoglycosides — drug classes typically reserved for more serious or resistant infections.
