Glaucoma care is getting a makeover — and eye drops are no longer the default
Glaucoma affects an estimated 4.22 million Americans, yet only about half have been diagnosed. Even among those who are, adherence to daily eye drop therapy is notoriously poor — just 56% of newly diagnosed patients fill enough drops in year one, and only 31% stay consistent after that. Add in side effects like ocular surface toxicity, skin changes, and even depression, and it's clear the "drops first" model has real limitations.
Enter interventional glaucoma (IG): a growing movement that repositions laser therapy, implantable drug-delivery devices, and minimally invasive glaucoma surgery (MIGS) as front-line options, with drops used only as adjuncts. A 2025 Interventional Glaucoma Consensus Treatment Protocol now gives clinicians a practical roadmap, starting with selective laser trabeculoplasty (SLT) and escalating through procedural options before reaching traditional filtering surgery. Patients are increasingly on board too — many now actively prefer procedures over lifelong daily drops.
