A large Swedish study found that patients with both atrial fibrillation and Alzheimer's disease who took NOACs experienced slower cognitive decline than those on warfarin or no anticoagulant. NOACs were also tied to lower risks of death, stroke, and fracture — without a significant increase in bleeding.
A nationwide Swedish study of over 7,000 older adults with both atrial fibrillation (AF) and Alzheimer's disease (AD) found that those taking non-vitamin K oral anticoagulants (NOACs) experienced modestly slower cognitive decline compared to those on warfarin or no anticoagulant at all. Published in the European Heart Journal, the study tracked patients for up to five years using annual Mini-Mental State Examination (MMSE) scores.
While the year-to-year difference was small, it compounded meaningfully over time — NOAC users averaged an MMSE score of 15.3 after five years, versus 14.3 for those on no anticoagulant. Researchers suggest the benefit may stem from fewer cerebral infarctions, better cerebral blood flow, and possible anti-inflammatory or amyloid-modulating effects.
