A major Danish cohort study is pushing back on a widely held assumption: that resistance to piperacillin-tazobactam (TZP), a commonly used antibiotic combination, automatically leads to worse survival in bloodstream infections (BSIs). Analyzing over 34,000 patients with E. coli or K. pneumoniae BSIs between 2018 and 2024, researchers found that while TZP-resistant infections appeared deadlier on the surface, that difference disappeared once patient health factors were properly accounted for.
The key insight? Sicker patients are more likely to both harbor resistant bacteria and die — making resistance look like the culprit when it may not be. After adjusting for comorbidities, hospitalization history, and other clinical factors, the mortality risk between resistant and susceptible infections was essentially the same.
